The completed form must be submitted to any Takaful myCare Centres, Takaful Retail Centres or Branches of Takaful Malaysia only. For new business submission/application for a Takaful plan, the completed Credit Card Debit Form must be submitted together with the proposal form to any Takaful myCare Centres, Takaful Retail Centres or Branches of Takaful Malaysia only. For existing certificate holder, the completed Credit Card Debit Form must be submitted together with Endorsement Form – Non-Financial. The company has the right to charge customer for any / total outstanding due(s) for the certificate enrolled / assigned with payment method as Credit Card Standing Instruction to ensure that the certificate is active and customer enjoys protection from Takaful Malaysia. No receipt will be issued. Cardholders must check their monthly credit card statement. For rejected/unsuccessful billing, please refer to Credit Card issuing bank for clarification as the explanations for the failed billing/deduction are solely meant for the affected Credit Card holder only. Maybank Auto Debit form can be downloaded from Takaful Malaysia website or available at any Takaful myCare Centres or Branches of Takaful Malaysia. Please complete the Maybank Autodebit form with details of certificate owner and details of Maybank’s account holder. For new business/new application, please leave the "Bill Reference No." blank. We will assist to complete it with the correct certificate number upon approval of the proposal. For existing certificate, please write clearly on each Maybank Autodebit form a CERTIFICATE ONLY. The company has the right to charge customer for any/total outstanding due(s) for the certificate enrolled/assigned with payment method as Auto Debit to ensure that the certificate is active and customer enjoys protection from Takaful Malaysia. Customers are advised to write 00 or leave it blank at the “Limit (Next Ringgit)” field. This is to avoid any disruption of billing in the event that the contributions amount stated differ from the actual contributions or the contributions stated are revised due to changes from time to time as advised by Takaful Malaysia, the Company is authorised to amend where necessary and debit the actual contributions accordingly. For new business submission/application for a Takaful plan, the completed Maybank Autodebit Form must be submitted together with the proposal form to any branch or Takaful myCare Center of Takaful Malaysia. For existing certificate holder, the completed Maybank Autodebit Form must be submitted together with Endorsement Form – Non-Financial. RM 0.50 will be charged to the account holder for each successful transaction. No receipt will be issued. Account holders must check their monthly bank statement. The completed form must be processed and authenticate by Maybank. Please call 1-300-88-6688 to make payment. RM0.50 will be deducted for each successful transaction. No receipt will be issued.
